If you have medical history in our clinic and medical ID number in our clinic, you can check in from your PC at home, smartphone or cellular phone (reservation is not available).
※Online check-in is available in 9:20am-11:30am / 3:20pm-5:30pm.
※New patients must check-in on-site, not online.

- ・Health insurance card
- ・"Mother-and-baby" notebook (children only)
- ・Matters of grant approval or claimant certification for medial care (children, elderly, handicapped, etc.)
- ・Reference letter (if available)
- ・Drug history notebook
